Each enrollment packet will include Child’s Personal Rights, Parent Rights, Child’s Pre-Admission Health History Parents Report, Identification and Emergency Information, Consent for Medical Treatment Form, Immunization Requirement, Physical Exam Requirements / TB Test Requirement and Admission Agreement. All forms must be completely filled out and signed for each child as well as a onetime non-refundable Registration fee of $300.
